# Event risk register — editable working template

Copy this file for one event. Replace every bracketed field. The example rows are illustrations, not an assessment of your venue. Ask the venue and competent local safety leads to review safety-related entries and use the approved event safety and emergency plans for decisions.

**Event and edition:** [name / date]
**Venue / site:** [name]
**Register owner:** [person and role]
**Current version / reviewed:** [version / date and time zone]
**Next review trigger:** [milestone, changed condition, or incident]
**Linked approved plans:** [document titles and current versions]

## Working register

| ID | Cause or condition → possible event → consequence | People or task affected | Existing control and evidence | Open action and evidence needed | Owner | Due / review trigger | Escalate to | Status |
| --- | --- | --- | --- | --- | --- | --- | --- | --- |
| R-01 | The published entrance route changes → attendees arrive at a closed gate → delayed or inaccessible entry | Arrivals and access requests | Venue map version [x]; gate lead confirms route [date] | Walk the accessible route with venue lead; update public directions and staff brief after approval | [name] | Before attendee directions are sent; again if route changes | [venue decision role] | Open |
| R-02 | A critical supplier delivery is delayed → the planned room cannot open → a session is displaced | Programme and attendee communications | Delivery confirmation [reference] | Confirm an approved alternative room and message owner; record decision | [name] | [date / supplier status change] | [event director] | Monitoring |
| R-03 | [cause] → [event] → [consequence] | [who/what] | [control; proof it exists] | [action; evidence of completion] | [person] | [date or trigger] | [role] | Open |

Use a stable ID even when a risk is closed. Link to the detailed venue risk assessment, emergency plan, supplier method statement or technical plan when that document owns the actual control. Do not paste sensitive personal details into a widely shared register.

## Decision and change record

| When and time zone | Risk ID | New evidence or changed assumption | Decision and authority | Public/staff update needed | Action owner | Verification / next review |
| --- | --- | --- | --- | --- | --- | --- |
| [timestamp] | R-01 | [route change confirmed by venue] | [approved route / approver] | [event page, email, signage, check-in brief] | [person] | [walk-through result and date] |

## Review prompts

- What changed in the venue, audience, weather, programme, supplier plan, ticket volume or access needs?
- Are the recorded controls real, current and understood by the people using them?
- Which open action needs a named decision before doors, and who can make that decision?
- Do the risk register, approved plans, public event information and staff brief agree?
- Has the owner recorded evidence of completion and set the next review trigger?

This worksheet helps a team track decisions. It does not certify a site, set legal requirements, choose crowd limits or replace a competent, site-specific risk assessment.
