Event incident report form template: record facts and follow-up
When something happens at an event, immediate care and the approved response procedure come first. A report then preserves what people actually observed, where and when it happened, what was done and who will follow up. The form should help a qualified lead review the incident; it should never delay emergency assistance or pretend to decide legal reporting duties.

Use the form after immediate response
Ask staff to raise the alarm, assist within their training and follow the venue-specific emergency plan first. HSE event guidance puts defined roles, public communication and coordination with emergency services in the response plan. A blank form does none of those jobs. Once the situation is stable, open one incident ID and assign a recorder and reviewer.[4]
Write the local date, time, exact zone or map reference, the type of incident as initially reported and the source of the report. If times are approximate, label them as such. Keep separate the time something occurred, the time it was discovered and the time the report was written. This matters when later updates fill gaps.
Separate observations, accounts and unknowns
Use short, neutral statements: what was seen or heard, by whom, and what was still unknown. Record a witness name/contact only when needed and permitted, and keep their account in their own words with the time it was obtained. Avoid a box that invites an untrained recorder to assign fault or diagnose an injury. OSHA recommends looking for causes and corrective actions rather than blame during a later investigation.[2]
| Good field | What it captures | Avoid |
|---|---|---|
| Location and time | Specific place and observed sequence | Vague “at the venue” |
| Immediate condition | What was visible or reported | Medical diagnosis by an unqualified person |
| Actions and handoff | Who was contacted, what they did, when | Assuming a call was made |
| Unknowns | Facts awaiting confirmation | Filling blanks with a guess |
If there is a photo, CCTV reference or equipment record, note where authorized reviewers can find it, who controls access and whether it has been preserved. Do not paste sensitive images or personal details into a shared event chat. Preserve the original record and add dated corrections rather than silently rewriting a witness account.
Record decisions, referrals and review
Include immediate actions, assistance or service referrals, the person notified, and the time of each handoff. Add a follow-up owner, due point and status so an unresolved hazard or attendee concern does not disappear after a shift change. If a separate safeguarding, medical or security procedure applies, reference its case ID and route the details to the authorized lead, not to a general event folder.
The leading Google result offers a downloadable incident PDF and suggests a witness signature and insurance copy. Those may suit that publisher’s context but are not universal requirements. This original template makes the witness field optional and asks the organizer to check its insurer, venue and local reporting rules. UK HSE notes that some workplace incidents require separate reporting and that personal details should be stored confidentially. Requirements vary by jurisdiction and incident type.[1][3]
Store securely and use the record to improve
Restrict access to people who need the record for response, investigation or required reporting. Set retention and deletion under the organization’s policy and applicable law; do not invent a universal number of years. Keep a version history if the account changes. The review can identify contributing conditions and preventive actions without turning the first report into a verdict.[2][3]
A printable or Markdown form is useful when connectivity fails, but decide in advance where completed forms go and who transcribes or secures them. Test that staff can find the blank form and the escalation contact before doors open. The download below provides neutral prompts and an action log, not an approved regulatory filing.
