Event staff expense claim form: receipts, approval and payment
When a crew member pays an event cost personally, the team needs a claim that says what was bought, why, which event it belongs to and what happened next. Use the editable form below for costs already incurred. A proposed purchase belongs in a separate request; an approved event budget alone does not approve each reimbursement.

The form has an event and claimant header, itemized costs, receipt references, an advance field, a review decision and a payment reference. Copy it for each claimant. Agree eligible categories, evidence, submission route and payment timing in your own policy before asking staff or volunteers to spend. This template does not set those rules for you.
Record the cost that was actually paid
For each line, enter the purchase date, item, event purpose, category, currency, amount and secure receipt reference. Link any prior approval that your policy requires. Do not turn a quotation into an expense already paid. If several currencies are involved, show a subtotal for each; finance should apply its approved conversion method later rather than silently adding unlike amounts.
- Identify the person who paid, their role and the event edition.
- Show any advance or direct payment so the same cost is not paid twice.
- Mark missing evidence or an exception for a named reviewer instead of guessing eligibility.
Review a claim without losing the decision
The event owner can confirm that the item served the event; finance can check the policy, evidence, duplicate claims and any advance. Record approved amount and currency, return for clarification or decline with a reason and decision date. A claimant should know which question remains open and where to send the answer. Keep the payment reference and status separate from the approval: a signed form is not proof that money was sent.
Keep sensitive details in the right place
Restrict the claim and receipts to the people who need them. The shared form needs a payment reference, not a full bank account or card number. Store payment instructions in the approved finance system, and use its retention and correction rules. Staff, contractors and volunteers may be treated differently under local law and organizational policy; ask the appropriate finance or legal lead to review those differences.
