Event volunteer shift change request and confirmation template
A volunteer who can no longer work an assigned event shift needs a clear way to request a change. A request is not a new assignment. Use one coordinator to review the affected post, record the decision and tell everyone who relies on the rota.

Copy the sheet for one event edition. Its request portion captures the current and proposed shift; its decision portion tracks coverage, approval, rota version and confirmation. Use a private contact route for personal circumstances. The shared rota only needs the operational outcome.
Ask for the exact shift change
Record the volunteer reference, current role, location, date and time, and the change they want: release from a shift, a different slot or a proposed swap with another person. Ask whether the proposed replacement has actually agreed to be considered. Do not treat a name in the form as that person’s acceptance. Include a contact route and the event-specific response expectation, if your coordinator can meet it.
Review coverage before making a decision
Compare the request with the current rota and role brief. Check the proposed person’s availability, any approved role requirements and whether moving them leaves another post uncovered. For a partial-shift change, identify the exact handover time and supervisor. If the role has a credential or task-specific requirement, the responsible lead confirms suitability; the form does not certify anyone. Mark the request pending while those checks remain open.
Update one authoritative rota and notify both sides
The coordinator records approved, declined or an alternative offered, with the decision time and owner. For an approved change, update the current rota first, then send the affected volunteers and supervisors the new role, location, arrival time and reporting contact. Confirm that the incoming person accepted the final assignment. If the change is declined, tell the requester which shift still stands and where to discuss a further option. Keep old rota copies identifiable so a stale screenshot is not mistaken for the current instruction.
Close the open loop before event day
The register stays open until the decision, rota update and notifications are recorded. A request with no confirmed replacement remains an uncovered-position problem for the coordinator, not a completed swap. At the next staff briefing, verify the affected supervisor has the current assignment and handover detail.
